Insurance claim requirements vary across insurers and policies. Please check with your insurer before your appointment to confirm the requirements for your claim. Receipts are available upon request. Please let our reception team know.

If your insurer requires any form to be completed by our physician, please let us know when making your appointment. Do bring the insurer’s printed form to your consultation and complete all sections intended for the patient beforehand. We are unable to assist with retrospective requests post-consultation due to compliance.

Completion of any insurance form remains subject to the patient’s clinical condition, available medical information, and the physician’s professional judgement.

因保险公司及保单的理赔要求各有不同，请务必在预约看诊前向您的保险公司确认理赔所需文件。诊所可提供收据，请告知前台。

如保险公司要求由医师填写相关表格，请在预约看诊时提前告知。看诊当天请务必携带保险公司提供的纸质表格，并提前填写好所有应由患者本人填写的部分。看诊结束后才提出的相关要求，恕无法追溯办理。

医师将根据患者实际情况、现有医疗资料，以专业判断决定是否适合填写保险表格，以及可提供的相关内容。

费用均依诊所统一标准计费。
